High Intensity Supports

SeenCare High Intensity Supports are provided where a NDIS participant has any of the listed complex care needs included in their service plan.

SeenCare staff can work with you, your carers, your support coordinator, NDIS plan manager, health, or disability providers to discuss the services and supports you need at home.

Seen Care is registered by the NDIS Commission to deliver support related to

  • Tracheostomy management
  • Ventilator management
  • Urinary catheter management
  • Complex bowel care
  • Enteral (Naso-Gastric Tube – Jejunum or Duodenum) Feeding and Management
  • Complex wound management
  • Subcutaneous injections
  • Severe dysphagia management

Not sure if your needs can be covered?

You can contact the SeenCare team to discuss other complex care needs you have, impacting a hospital discharge process or level of care needed at home.

We understand that all people are different and their complex health needs change or may involve more than one condition.
