SeenCare was started by a group of doctors with expertise in general practice (GPs) and hospital care.
The doctors began SeenCare to address the delays in discharge that they identified people with complex needs and disability were facing due to the level of home care needed.
SeenCare SIL participants benefit from our unique integrated model that includes doctors.
Medical expertise in the team has been included due to many people with disabilities not having a local link with a GP in their area.
SeenCare doctors do not replace the participant’s own GP, instead the team will consult with the local GP if needed.
SIL participants can choose to link with SeenCare GPs or not as this is an extra service if needed. All consultations are bulk billed through Medicare.
SeenCare staff receive support from the GPs in the team who provide clinical governance and guidance on participant health conditions and management of any changes in the heath care needs of the participant.
Yes – Due to the high demand for bulk billing consultations we only provide GP services to SeenCare SIL participants
SeenCare GPs bulk bill for the support provided to SeenCare SIL participants who use this optional service.
Yes – we encourage this and SeenCare welcome shared care arrangements with participant’s GP for holistic care.
All SeenCare participants have an emergency care plan that is used when needed.
This is followed when nursing or disability support workers identify urgent escalation signs or symptoms.
