SeenCare understand the challenges NDIS participants face in accessing nursing services at home.

We set up SeenCare to make this as easy as possible so eligible clients can receive high-quality care at home in Melbourne.

Our experienced nurses work in close partnership with the rest of our team, including bulk-billing GPs and disability support workers and.

Our team can discuss any needs not on this list and how a care plan could be supported.

Tracheostomy Care

Comprehensive management of tracheostomy tubes, suction, humidification.

Emergency response delivered safely in your home.

Peg and Swallowing Assistance

Peg feeding and maintenance.

Meals assistance for swallowing difficulties.

Training for support workers.

Ventilation Management

Comprehensive management for people receiving long term mechanical ventilation support.

Worker’s training tailored to individual needs and equipment and specific health condition.

High-risk Medication Management

Daily administration of medications including subcutaneous injections.

Monitoring for side effects and liaison with GP.

Management of high risk drugs.

Referrals for additional support

Wound and Pressure Ulcer Care

Wound cleaning and dressing changes.

Preventative care and ongoing management.

Pressure ulcer assessments and care plans.

Catheter and Stoma Care

Insertion, maintenance and monitoring of catheters.

Assistance and support with stoma care.

On-call help with catheter and stoma issues.

Diabetes Management

Blood glucose monitoring.

Insulin administration.

Comprehensive diabetes care plans.

Severe Dysphagia Management

Comprehensive management for people living with severe dysphagia.

Worker’s training tailored to individual needs and equipment and specific health condition.

Epilepsy and seizure management

Comprehensive management for people living with epilepsy and seizures.
